Biologics and targeted therapies
TNF inhibitors (adalimumab, etanercept, infliximab), IL-17 and IL-23 inhibitors, B-cell and T-cell therapies, JAK inhibitors, and MS disease-modifying therapies. These are prescribed and managed by your specialist. What matters between visits is infection watchfulness, vaccine timing, injection-site issues, and honest reporting of how you're actually doing.
Corticosteroids
Prednisone works fast and costs something. Insomnia, irritability, anxiety, appetite and weight changes, blood sugar elevation, and — at higher doses — genuine mood instability. If your mood shifted after a steroid burst, that is a known pharmacologic effect, not weakness.
Psychiatric medications in a chronic-illness body
Antidepressant choice should account for pain, sleep, appetite, and your existing regimen — duloxetine has evidence in chronic musculoskeletal pain; low-dose amitriptyline or gabapentinoids can help pain-related sleep disruption; bupropion is often chosen when fatigue and low motivation dominate. ADHD treatment can continue alongside chronic illness, with attention to blood pressure, heart rate, sleep, and appetite.

Causes of chronic illness fatigue
Inflammation is one driver of fatigue, but it's far from the only one, and in many people several things overlap at the same time. Some of the possibilities worth keeping on the table:
- Chronic inflammation — conditions like rheumatoid arthritis, lupus, and inflammatory bowel disease keep the immune system releasing cytokines. That sustained immune response has a real metabolic cost and directly drains physical energy.
- Mitochondrial dysfunction — in ME/CFS and fibromyalgia, cells appear to struggle to convert nutrients into usable cellular energy efficiently, which is part of why rest doesn't restore what you'd expect it to.
- Impaired oxygen delivery — anemia, chronic kidney disease, and heart failure limit how well oxygen-rich blood reaches muscle and tissue, so you tire far faster than the activity should warrant.
- Anemia specifically — iron deficiency, chronic disease anemia, B12 or folate deficiency, or GI blood loss from long-term NSAIDs.
- Hormonal and metabolic fluctuation — swinging blood sugar in diabetes and a slowed metabolic rate in hypothyroidism both disrupt how the body regulates and spends its daily energy supply. Adrenal issues belong here too.
- Fragmented sleep architecture — sleep apnea, restless legs, and nighttime pain repeatedly pull you out of the deep, restorative stages of sleep. You can spend eight hours in bed and get very little of the sleep that actually repairs you.
- The energy cost of pain — unmanaged, ongoing pain keeps the nervous system hyper-alert. That vigilance runs in the background all day and consumes a large share of your baseline energy before you've done anything else. Pain causes fatigue, plainly, and treating the pain is often part of treating the fatigue.
- Dysautonomia and POTS — fatigue that worsens with upright posture, often alongside lightheadedness and palpitations.
- ME/CFS and post-viral syndromes, including long COVID — the defining feature is post-exertional malaise, where symptoms worsen for a day or more after even modest activity. This is not simply inflammation, and it is not deconditioning; pushing through can make it worse, which is why pacing matters.
- Medication effects — sedating antihistamines, beta blockers, gabapentinoids, opioids, muscle relaxants, some antidepressants, steroid crashes.
- Depression, anxiety, and ADHD — real contributors, and treatable ones, though they should not be the first or only explanation offered.
- Undernutrition, dehydration, deconditioning, and the sheer cognitive load of managing a chronic illness.
Working through these usually takes more than one visit, and the answer is often a combination rather than a single cause. Basic labs rule in or out the treatable pieces; the pattern of your symptoms — especially whether activity makes things worse afterward — helps point toward the rest.
It's worth saying plainly how debilitating this kind of fatigue can be. It isn't ordinary tiredness that a good night's sleep fixes. It's waking up already depleted, showering and needing to sit down afterward, and rationing the day between what has to get done and what will have to wait. Advice to "just move more" lands badly here, because movement is exactly the thing the fatigue and pain are limiting. When even gentle activity costs more than it returns — or triggers post-exertional malaise a day later — telling someone to exercise their way out of it isn't a plan.

Depression, anxiety and ADHD with chronic illness: real, common, and treatable
Depression and anxiety are roughly two to three times more common in people with chronic illness than in the general population, and that isn't only a psychological reaction to bad news. Inflammatory cytokines act directly on the brain to produce fatigue, withdrawal, and low mood. Pain fragments sleep. Steroids destabilize it further. Unpredictability keeps the nervous system braced.

- A body doing immune work all day has less capacity. Baseline fatigue isn't laziness — it's physiology.
- Function will vary week to week. Planning around your worst day makes life smaller; planning around your best day guarantees crashes. Plan around your average.
- Pacing beats push-and-crash. Stop the activity while you still have something left.
- Grief is appropriate. Losing the version of yourself who didn't have to think about any of this is a real loss and deserves real attention.

How to advocate for yourself and stay organized with chronic illness
Nobody has a complete picture of your health except you. Every office holds a fragment. The patients who get the best care aren't the loudest — they're the ones who arrive with documentation and a clear question.
A practical system
- Keep one running document with your diagnoses, dates, current medications with doses, past medications and why they were stopped, allergies, and every specialist's name.
- Track symptoms in a simple daily log: pain 0–10, fatigue 0–10, sleep hours, stiffness duration, mood, and anything unusual. Patterns are more persuasive than memory.
- Note what you were doing when a symptom hit — a flare after every steroid taper or every high-stress week is clinical information.
- Bring three questions to each appointment, written down, in priority order. Ask the most important one first.
- Request your labs and imaging reports through the portal and keep them in one folder. You will need them for a new provider someday.
- Ask directly: 'What is this test for, what would change based on the result, and who is following up?'
- If you're told something is 'just stress,' ask what has been ruled out. Anxiety and autoimmune disease can absolutely coexist — one doesn't disprove the other.
- Bring someone to appointments that matter, or ask permission to record, when brain fog makes retention hard.
